- Contact us at support@formula.health with the subject line “Refund Request”
- Include your contract reference, purchase date, and reason for requesting a refund.
- Our team will review your request in accordance with your contract terms and respond within 5 business days.
- If approved, refunds will be processed to the original method of payment within 10 business days
